Answer :
In the Name of Allah, the Most Gracious, the Most Merciful As-salāmu ʿalaykum wa-raḥmatullāhi wa-barakātuh 1) The clear discharge you experience whenever you feel like you need to go to the toilet to pass stool is impure. Due to your circumstances, you do not have to change or wash your underwear and trouser every time you pass the clear discharge. You may keep on one underwear and trouser throughout the day, even if they are soiled each time you experience a discharge. However, at the time of Ṣalāh you should remove the soiled clothes and wear clean clothes after cleaning the area of your body soiled by the discharge. After performing Ṣalāh, you may remove the clean clothes and change back into the soiled clothes. You should minimize the amount of impurity on your clothes by placing, for example, toilet paper or a pad where you experience the discharge. If your clothes become extremely soiled after several discharges, it is advisable that you try to change or wash them at least one or two times a day. As for washing yourself (making Istinjā of the impure area) after the discharge, this would be necessary when the amount of the discharge has reached the size of the hollow portion of a person’s palm. 2) A wet dream is a dream by which a person is sexually aroused. When you awaken after having a wet dream, if you do not see or feel any wetness, it is not necessary to make Ghusl (take a ceremonial bath). Nevertheless, if you do take a Ghusl, this would be as a matter of precaution, although it would not be necessary to do so. However, if you see or feel any wetness, you should make Ghusl and wash or change your soiled clothes.
